The only distinctive difference is in the legs, and for obvious reason: The 250 lb. woman has more pronounced thighs for the simple fact they are responsible for transporting her weight. More interesting is the underdeveloped inner calves. This is because the person is forced to walk on her ankles due to the excessive weight impacting her joints.
The implications of being overweight have personal meaning to me: In January 2016, I watched my wife crumble after her mother died unexpectedly of obesity-related causes. She was 5'2" and 276 lbs. Granted, fat doesn't kill directly, but as is clear from the pic on the right, being overweight greatly increases the risk of potentially fatal illnesses such as colon cancer, heart disease, diabetes, stroke, and liver disease.
